Ministry Reimbursement Form
Use this form to request reimbursement for approved ministry-related purchases and expenses. Please have your receipts and supporting information ready.
Submitting this form does not guarantee reimbursement. All requests are subject to review and approval.
Benevolence Assistance Form
If you are experiencing a financial hardship or urgent need, complete this confidential form to request benevolence assistance.
All requests are reviewed confidentially and are subject to available resources and established assistance guidelines.
Facility Usage Form
Request the use of a Central Union facility or meeting space for a ministry gathering, meeting, celebration, or approved community event.
Submitting a request does not reserve the facility. The requested date and event must be reviewed and approved.
Donation Tax Letter Form
Request a copy of your annual contribution statement or donation tax letter for your financial records.
Please allow adequate processing time and make sure your contact information is accurate.
